Apply to Camp
To apply to one or more of the Soccer Centers, please follow these steps:
- Download the Center of Excellence application you are interested in attending. Print, and fill out the information requested. ** The Medical Form with signatures AND a copy of your insurance card back and front MUST be on file for you to be registered and eligible to play in camp!**
- Mail the application, medical form, and your deposit to the address indicated on the application.
- If you CANNOT download the application, please e-mail jef7r@virginia.edu and request that it be mailed to you. An application will be sent to you as soon as possible.
